Clinical Info

Screening test for gestational diabetes mellitus.
Perform a 50-g glucose load test (nonfasting), with plasma glucose measurement at 1 h post ingestion.
This test is typically performed at 24–28 weeks of gestation in individuals not previously diagnosed with diabetes.

Collection Instructions

1. Fasting is not required.
2. Patient should be instructed to refrain from smoking and drinking coffee just before and during test.
3. Patient is given a 50 g dose of Glucola (patient must drink entire beverage within 5 minutes).
4. Draw blood specimen at 1 hour post Glucola ingestion.

Transport Instructions

Refrigerate. Specimen must arrive within 24 hours of draw.

Result Interpretation

ADA diagnostic criteria for gestational diabetes mellitus following 50-g glucose load test:

1 Hr  post-glucose load:  70 - 134 mg/dL
 
In the absence of clear evidence supporting a cutoff of 135 mg/dL versus 140 mg/dL for the 1-h glucose screening test, it is suggested that health care providers select one of these as a single consistent cufoff for their practice, with factors such as a community prevalence rates of GDM consideration in the decision.

If the plasma glucose level measured 1 h after the load is ≥135 mg/dL, proceed to a 100-g oral glucose tolerance test.
